  • Discharge Summary: The discharge summary card provided by the hospital comprising details such as the number of days you were hospitalised, recommended post-hospitalisation tests, follow-up visits, etc.
  • Medical Reports: Copies of diagnostic tests recommended, medical reports, blood tests, etc., undertaken during the pre-hospitalisation stage.
  • Bills & Receipts: All original bills, including hospitalisation bills stamped by the medical facility, bills of medical tests undertaken, medicines used during surgical procedures, bills generated by pharmacies during the course of the treatment, etc.
