  2. Fill out the form: As mentioned above, print your form and fill in the details required, such as your name, policy number, etc. You can get a hard copy of the form at a hospital desk or find a soft copy on your insurer’s official website and take a printout.

  3. Take a copy: After filling out and signing the form and attaching all the required documents, you should try to make copies of the forms for a smooth claim settlement process. Plus, it also serves as a backup if you lose your documents.

  4. Review and submit: Finally, cross–check your documents and go over the details entered. Once done, you can send your documents to the health insurance provider and wait for the settlement process to conclude.
